By the start hour of your slot in Ecuador time. Day (08:00–18:59 ECT) is USD 50. Night (19:00–07:59 ECT) is USD 90. The system calculates it — you cannot change it.

Yes. Uploaded files are not stored as public URLs. Only the assigned doctor, you, and authorized FirstCare staff can access them.
